Authorization Form - Direct Payment:
To change your banking information for commissions, please download the Direct Payment Authorization Form for Publishers and send all payment information to the Accounting Department at accounting@citylifestyle.com.

Proforma Invoice:
A proforma invoice serves to provide the client with a preview of upcoming charges prior to the official invoice date. Official invoices will not generate until your invoicing date.
Once details are filled in, go to file > download PDF Document to share PDF with your client.
