• Authorization Form - Direct Payment: To change your banking information for commissions, please download the Direct Payment Authorization Form for Publishers and send all payment information to the Accounting Department at accounting@citylifestyle.com.

• Proforma Invoice: A proforma invoice serves to provide the client with a preview of upcoming charges prior to the official invoice date. Official invoices will not generate until your invoicing date. Once details are filled in go to; file > download PDF Document; to share a PDF with your client.
